We provide a range of services and programs to support the physical, emotional, and social well-being of our participants. We are seeking a compassionate and dedicated Case Manager to join our team and contribute to our mission.

Salary: Competitive salary and comprehensive benefits package, including health reimbursement and matched retirement savings (salary range $75,000 to $85,000)
The Case Manager at Senior Coastsiders is responsible for providing comprehensive case management services to older adults, addressing their physical, emotional, and social needs. This role involves assessing individual needs, connecting seniors to appropriate resources, and coordinating care plans to promote well-being, independence, and community engagement. The Case Manager will also advocate for older adults, ensuring they receive the necessary support to thrive. Apply.
Key Responsibilities
- Client Assessments: Conduct intake assessments to evaluate the health, financial, and social needs of older adults using the Senior Coastsiders services.
- Develop Care Plans: Create individualized care plans that include services such as healthcare referrals, social services, transportation, housing assistance, and mental health support.
- Resource Connection: Help seniors access community resources, including medical services, food assistance, financial aid, legal services, and home health care.
- Case Coordination: Collaborate with healthcare providers, social workers, family members, and community organizations to coordinate the delivery of services and care for older adults.
- Monitor Client Progress: Conduct regular follow-ups to assess changes in a client’s needs, adjust care plans as necessary, and ensure goals are being met.
- Crisis Intervention: Provide immediate assistance during emergencies, such as health crises or housing instability, connecting seniors with emergency services or temporary solutions.
- Documentation: Maintain accurate and up-to-date case files, documenting client assessments, service plans, communications, and progress notes in compliance with organizational policies and confidentiality standards.
- Advocacy: Act as an advocate for seniors, ensuring their needs and preferences are considered in decision-making processes with healthcare providers, social services, and family members.
- Program Development: Collaborate with Senior Coastsiders staff to design programs that enhance the quality of life for older adults, including wellness programs, educational workshops, and social activities.
- Family Communication: Provide information and support to family members and caregivers, keeping them informed about the care and progress of their loved ones.
- Assist with Benefits Applications: Help older adults apply for benefits and services such as Medicare, Medical, CalFresh, and affordable housing applications.
Qualifications
- Education:
Bachelor’s degree in Social Work, Gerontology, Human Services, Nursing, or a related field. A Master’s degree in Social Work (MSW) or a related discipline is preferred. - Experience:
- Minimum of 2 years of case management experience, preferably working with older adults, in settings such as senior activity centers, healthcare, or social services.
- Experience with public benefits programs, and community resources is a plus.
- Certifications:
- Licensed Clinical Social Worker (LCSW), Certified Case Manager (CCM), or equivalent certification is preferred.
- CPR/First Aid certification is a plus.
- Skills and Competencies:
- Strong communication and interpersonal skills, with the ability to establish rapport with older adults and their families.
- Ability to speak Spanish is a plus.
- Knowledge of aging-related services, healthcare systems, and community resources for seniors.
- Excellent problem-solving and crisis management skills.
- Ability to work independently, manage multiple cases, and prioritize workload effectively.
- Compassionate, patient, and understanding of the diverse needs of older adults.
- Strong organizational skills and attention to detail in care planning and documentation.
- Proficiency in Google Suite, case management software, and online benefits application platforms.
Physical Requirements
- Ability to sit, stand, and walk for extended periods.
- Occasional lifting of up to 25 lbs.
- Ability to assist clients with mobility challenges when necessary.
Work Environment
- The position is primarily based at Senior Coastsiders but may involve occasional visits to clients’ homes, healthcare facilities, or other community organizations.
- The Case Manager must be able to work in a dynamic environment where multiple programs and services are provided simultaneously.
